Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Wit: 10 (2d8+3) piercing damage plus 8 (1d1o+3) poison damage. A successful DC 15 Constitution saving throw halves the poison damage.
Jail Slam. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 10 (2d8+3)

Putrid Spew (Recharge 5-6): The Putrescent Annelid regurgitates a torrent of poisonous bile in a 15-foot cone. Each creature in that area must make a DC 15 Dexterity saving throw, taking 34 (8d8) poison damage on a failed save or half as much damage on a successful one.
Blood Drain. When the Putrescent Annelid hits a creature with its Bite attack, it may attach itself to the target. f attached, at the start of each of the Annehds turns, the target loses 7(2d6) hit points due to blood loss. Ohe Annelid regains hit points equal to half the damage dealt. A creature may attempt to detach the Annelid on their turn by using an action to make a DC 14 Strength (Athletics) check, removing the Annelid on a success.
Aggressive. As a bonus action, the Putrescent Annelid can move up to its speed toward an enemy it can detect.
Blood spray. The Putrescent Annelid has advantage on attacks made against creatures that have less than their maximum hit points.
The Putrescent Annelid prefers ambush tactics, lurking unseen in stagnant waters or shadowy pools until prey ventures too close.

In forgotten boglands where the water festers stagnant and dark magic seeps like a slow poison into the roots of the world, the first Putrescent Annelid was spawned. It was a small, mundane leech, nourished by the corrupted runoff of a blighted alchemical laboratory. Over time, its form twisted, and its hunger grew insatiable, turning from simple sustenance to a craving for life essence itself.
The locals soon discovered livestock drained and desiccated, their bodies bloated with toxins. Hunters set out to end the menace, but few returned, their empty husks joining the mounting toll of its victims. Sales spread of a creature lurking beneath murky waters, a nightmare made real, cloaked in shadows, waiting patiently for prey to wander near its lair.
Now known as the Putrescent Annelid, this foul being lurks in stagnant waters, corrupted marshes, and subterranean pools. Its presence is heralded by the stench of decay and the slime-slick trails it leaves behind. Many who encounter this abomination and survive carry the scars of its bite, forever tainted by its vile venom...
