Adaptive Form. At the end of its turn, the guardian rolls a d4 to determine its next form, which it assumes at the start of its next turn. The guardian gains a damage vulnerability tied to its upcoming form until the transformation occurs.

  • Radiant Storm: Creatures within 15 feet of the guardian must succeed on a DC 18 Constitution saving throw or take 21 (6d6) radiant damage and be blinded until the end of their next turn.
  • Crystalline Knight: The guardian’s AC increases to 22, and it gains 25 temporary hit points. Its melee attacks deal an additional 10 (3d6) slashing damage.
  • Swarm of Butterflies: The guardian becomes incorporeal and can pass through other creatures and objects. It can occupy another creature's space and automatically deals 14 (4d6) radiant damage at the start of that creature’s turn.
  • Verdant Bloom: The guardian releases a burst of magical pollen in a 20-foot radius. Each creature in the area must succeed on a DC 18 Wisdom saving throw or be charmed until the end of the guardian’s next turn. Charmed creatures cannot willingly move or attack.

Illusory Doubles. When the guardian is targeted by an attack, roll a d6. On a 4 or higher, the attack targets an illusory duplicate instead of the guardian. The duplicate is destroyed after the attack resolves. The guardian can have up to 3 duplicates at a time.

Magic Resistance. The guardian has advantage on saving throws against spells and other magical effects.

Future Glimpse. A creature that uses an action to study the guardian or gazes into the Reflecting Pools can make a DC 18 Insight check. On a success, they choose one of the following boons:

  1. Learn the guardian’s next form and its associated damage vulnerability.
  2. Ignore the effects of Illusory Doubles for themselves and their allies until the end of the creature’s next turn.

Actions

Multiattack. The guardian makes two Radiant Strike attacks.

Radiant Strike. Melee Weapon Attack: +10 to hit, reach 10 ft., one target.
Hit: 22 (3d10 + 5) radiant damage.

Pulse of Light (Recharge 5–6). The guardian emits a radiant pulse in a 30-foot radius. Each creature in the area must make a DC 18 Dexterity saving throw, taking 36 (8d8) radiant damage on a failed save or half as much on a successful one. Creatures that fail the save are also pushed 15 feet away from the guardian.

Legendary Actions

The Prism Nexus Guardian can take 3 legendary actions, choosing from the options below. Only one legendary action can be used at a time, and only at the end of another creature’s turn. The guardian regains spent legendary actions at the start of its turn.

Mirror Step. The guardian teleports up to 40 feet to an unoccupied space it can see.

Prismatic Burst. The guardian creates a burst of light in a 10-foot radius around a point it can see within 60 feet. Each creature in that area must succeed on a DC 18 Constitution saving throw or take 18 (4d8) radiant damage.

Reform Illusions (Costs 2 Actions). The guardian replenishes all of its illusory doubles.

The Prism Nexus Guardian is a radiant construct crafted by Pride to defend the heart of the Prism Gardens. Its shimmering, crystalline form constantly shifts, reflecting the gardens’ adaptive and ethereal nature. Each phase of its form embodies a unique magical theme, blending raw power with tactical ingenuity.

Designed as both protector and ultimate test, the Nexus Guardian disorients intruders with radiant energy, illusory doubles, and unpredictable transformations. It reacts with uncanny awareness, exploiting weaknesses while forcing challengers to adapt quickly. Facing the Nexus Guardian demands foresight, teamwork, and resilience, as it embodies the Prism Gardens’ lessons of adaptability and critical thinking.
